I've used Jotform (http://www.jotform.com ) to create a survey for my blog. They have a great online tool for creating forms. I didn't think to put it in the sidebar until I read your question. Did it now, and worked like a charm, just need to edit the width of the survey to make it look good, but that should be relatively easy. You can see what it looks like (and answer the survey) at my blog (http://blog.solata.com ).

Thanks for the suggestion! I ran a test run and it worked like a charm collecting data. Does it have any functionality to put the results back up in graph form on the blog?

Glad you found it useful. I know you can export data to excel, etc., but I think the summary output needs to be manually created from the raw data you have obtained. I've suggested this as a feature enhancement to the creators.

May not be what you're looking for if you want the survey completely embedded in the webpage, but if you're looking for small surveys with real-time charts, try out www.livemetric.com.

Looks like a nice survey product. I am looking for something that embeds both the survey and the resulting graphs into the rails (or body), which live metrics doesn't seem to do. Thanks for the suggestion.
